Clinical research has a quiet cost problem that rarely shows up in budgets, until it’s too late. Often, the highest hidden cost isn’t the drug or the platform. It’s the measurement strategy.
The real question
The question isn’t “what can we measure?” It’s “What must we measure to make the decision defensible?”
Across Phase II–III trials and translational studies, data volume is exploding. But the pain isn’t the number of datapoints. Most studies don’t fail because they collect too little data. They fail because they collect low-information data at the wrong time— measurements that add cost, noise, and burden without improving inference.
Three recurring “measurement failures”
- Low-information markers — Biomarkers that are redundant, weakly tied to the mechanism, or mostly normal in the target population. They increase variance more than they improve the signal.
- Timing blind spots — Sampling schedules that miss the window where the signal actually appears: peak response, early warning shifts, or meaningful trajectory changes.
- Over-collection — A growing list of “nice to have” measurements that inflate cost, participant burden, protocol deviations, and missing data, without strengthening the final decision.

Why this is more expensive than you think
Over-measurement doesn’t just waste money on lab tests. It creates second-order costs:

“But we already have solutions…” (Not at the decision layer.)
Yes, there are real methods: optimal design (Fisher information, D-optimality), PK/PD sampling design, VOI/EVSI thinking, adaptive monitoring, and even ML systems that flag low-yield labs.
The gap is integration.
Most teams don’t fail because they lack tools. They fail because these tools rarely translate into a decision-ready, protocol-ready, budget-ready package that a PI, sponsor, lab lead, and finance/IRB reviewer can all defend.
You may get optimization outputs that don’t map cleanly to protocol constraints, or generic advice to “simplify endpoints” without mechanism-grounded justification.

How we compute “information per cost” (without heavy math)

- Does this measurement materially reduce decision uncertainty? (signal sensitivity, identifiability, expected variance reduction)
- Does it change an action? (go/no-go, dose, endpoint choice, cohort expansion, follow-up timing)
- What is the marginal cost and burden? (assay cost, visit complexity, missingness risk)
- What happens if it fails? (sensitivity analysis: if the marker is noisy, delayed, or missing, does the conclusion change?)
Practically, this is VOI/EVSI thinking translated into protocol terms: keep measurements whose marginal information value exceeds their marginal cost/burden/risk, and document the rationale for each “keep / drop / optional” decision.
A concrete (anonymized) example
In a translational program with a dense exploratory biomarker panel, the team was planning frequent sampling across a wide window “just in case.” After a mechanism-anchored stress test, two changes drove most of the value:
- Dropped a subset of low-yield exploratory markers that added noise without improving inference,
- Shifted sampling toward the likely response window instead of evenly spacing visits.
Result: the protocol retained decision interpretability while reducing measurement burden and direct assay spend (and, just as importantly, reducing missingness risk). The “win” wasn’t collecting less data. It was collecting the right data at the right time.
